  2. Scan the QR code on the form. If you are already on your phone, tap the Pulse link instead.
  3. Keep the form page open until it says the phone check worked.
  4. Return to the form and submit.

What Stays On Your Phone

The form does not get your raw motion data, raw sensor readings, Wi-Fi name, Bluetooth device name, phone call details, contacts, device serial number, or a trail of where you have been.

The form gets a short result for this one check, such as whether the phone check worked and a simple status.

If It Does Not Work

  • Refresh the form page and try again. Use the newest QR code. Old codes expire.
  • Make sure the phone has internet access and that the Pulse app can open.
  • If it still does not work, contact the company that sent you the form. They control the form and any backup review option.
